New Service Forms

Please make sure to print CCWSC Service Application and Agreement and fill out completely before sending them into the CCWSC.  Please include a copy of your Warranty Deed with the Application and Agreement. Include your checks or money orders for new service, deposits have to be paid on a separate check or money order.  Service will not be started until payment is received.

You can mail or drop off application and payment at our office at 416 Carancahua Blvd., Palacios, Texas 77465.   

Request for Discontinuance of Service Form

If you would like your service discontinued you will need to completely fill out and sign the below request. We can not voluntarily discontinue service until we have this form. If you re-connect your service you will be charged a $100.00 reconnect fee, $100.00 deposit and if anything is owed on the account will have to be paid up front including assessment fees if not up to date.
